In addition to providing the ingredients and instructions, please include:

1. Which procedure you had
2. How long after surgery you began eating this recipe
3. If you have them handy, please include nutrition facts (calories, protein, fat, carbs, sodium, fiber, cholesterol, sugar, etc.)

Also, if you know the instructions for procedures other than the one you had, please include them as well (i.e. after lap band surgery...., after gastric bypass..., after duodenal switch surger, after gastric sleeve surgery, etc.).
